Just FYI: I had the AOC monitor hooked up to the laptop when running that command.

I think this is a common problem with Intel Chipsets. I suggest you check this FAQ:
FAQ #1901: “Intel Chipsets Screen Resolution Problem”.

Thanks a lot for pointing me to this solution.

The first step says: 1.- Search the values of HorizSync and VertRefresh of your monitor.

How do I do that?

Furthermore, do I have to follow this step every time I connect a new monitor (a different one from what I have at home)?

If so, what do I do to get this bug filed correctly, so that with the next update or in future versions, this issue with Intel chipsets gets resolved.

I also have the same bug with same log as KushalSharma, eveything works correctly on Ubuntu 2D session but not in Ubuntu 3D or what so called.

i tried to fix it manually using xrandr but when i set the correct size/mode and rotation i see only white screen, only the task-bar does appear and if i clicked an item on the task-bar it refreshes very heavily , then i had to unplug the BraviaTV to get the screen restored.

setting the size/mode on Ubuntu 2D does work

the user Wannabegeek reported that in ubuntu 11.10 this issue doesn't occurs on the same hardware,
so the issue appears to be with the new driver in ubuntu 12.04
