squid3 with SSL

I want to create a derivative package of squid3 that includes --enable-ssl in it's configuration. I've built this manually to reverse-proxy an Exchange OWA server, but I want to create a Puppet module that can install and configure this without forcing the installation to do a full compilation.

1) If I create a derivative of an existing package, do you have a procedure for submitting that back to the community?
2) What changes should I make to the debian control and change files to leave the original attribution in place?
3) What is the procedure for getting such a package into the ubuntu package repositories?
4) If it is not preferred by the community to have that in the repositories, how do I host a small repo of my own that includes the new package?

You could create a PPA (Personal Package Archive). See http://developer.ubuntu.com/publish/apps/other-forms-of-submitting-apps/ppa/
