Use "All commands" dialog, select "merge" command, in the arguments filed insert full filename to patch with "Insert filenames..." control. Then press OK.

Is there any practical way other than "All commands"?

Mmm, I think not (yet). It's the bug https://bugs.launchpad.net/qbzr/+bug/430175. If you want to fix it I can guide you.
